INDICATION

• Indicated for the treatment of thyroid cancer that cannot be treated with radioactive iodine.

• Indicated by combined with everolimus to treat advanced renal carcinoma in which other drugs failed to be effective.

• Indicated for the treatment of hepatocellular primary liver cancer

DOSAGE

• The recommended dose of differentiated thyroid cancer is Lenvatinib 24 mg, given in two 10 mg and one 4 mg capsules orally, once daily, with or without food.

• The recommended dose of renal cell carcinomas is Lenvatinib 18 mg, given in a 10 mg capsule and two 4 mg capsules given in combination with 5 mg everolimus orally once daily with or without food.

• The recommended dose of first-line treatment of unresectable hepatocellular carcinoma is based on actual body weight

• If <60 kg then 8mg VO day

• If ≥ 60 kg then 12mg VO day

Administration :

• Lenvima 10mg capsule   should be taken at the same time each day.

• Follow the drug until the progression of the disease.

• Lenvima 10mg capsule   should be swallowed whole.

• The capsules were dissolved in a small glass of liquid.

• In 1 tablespoon of apple juice or water and embed the capsules in the liquid without breaking them or crushing them.

• Dip the capsules in the liquid for at least 10 minutes.

• The liquid is stirred for at least 3 minutes. Drink the mixture.

• Add 1 tablespoon of water or apple juice to the glass after drinking the mixture.

• Mix the contents several times and swallow the additional liquid.

Overdose : 

• Lenvima 10mg   has no individual antitoxin for overdose. From high binding to plasma proteins. Lenvima 10mg capsule  is not expected to be dialysable . Take patients to the emergency medical department or call the poison control helpline. 

MECHANISM

Lenvima 10mg   called as targeted therapy. In which it points something particular to cancer cells, therefore, it reduces the side effects caused by the repair of healthy cells. Lenvima 10mg capsule   by inhibiting two processes that allow cancer cells to develop

• Prevention of a protein that promotes cell division.

• Stop the VEGF receptor, which is necessary for angiogenesis, or the growth of a blood supply to the tumor. This rules out the source of the tumor's nutrients.

ADME

• The maximum plasma concentration is 1 to 4 hours after the dose.

• The effect with food is low in the degree of absorption.

• Human plasma proteins that bind approximately 98% to 99%.

• Metabolized in CYP3A and aldehyde oxidase. The metabolic pathway of lenvatinib is identified as enzymatic and non-enzymatic processes.

• The drug is excreted through 64% of feces and 25% of urine,

• The terminal half-life of Lenvatinib is 28 hours.

CAUTION

• Al usar  Lenvatinib,  hipertensión de  redujo en el 73% de los pacientes tratados con Lenvatinib en ensayos clínicos (la hipertensión de grado 3 fue del 44%); regular la presión sanguínea antes del tratamiento; controle la presión arterial después de 1 semana, luego cada 2 semanas durante los primeros 2 meses.

• Lenvima 10mg  causa eventos tromboembólicos arteriales ocurridos en el 5% de los pacientes tratados; suspender el fármaco después de un evento tromboembólico; suspenda para siempre el tratamiento después de un evento trombótico arterial.

• Cuando se usa  Lenvatinib,  se producen cápsulas de. Perforación gastrointestinal o fístula (2%); detener si el paciente experimenta una perforación gastrointestinal o una fístula potencialmente mortal; suspender permanentemente la terapia en pacientes con perforación gastrointestinal de cualquier fístula grave o de grado 3 o 4.

• Mientras esté en tratamiento informará complicaciones graves de hipertensión mal controlada.

• Discuta con el médico si está amamantando o planea amamantar. Evite amamantar durante el   tratamiento con Lenvima 10mg .

DOSIS PERDIDA

• Se dosis de Lenvima 10mg  y luego se toma la dosis como se acordó anteriormente. Si llega el momento de la siguiente dosis, cambie la dosis omitida y continúe con el programa de rutina. Evite tomar 2 dosis a la vez. Consulte a su médico para obtener más aclaraciones.
